Lustig therefore argues that sugar isn't merely a source of “empty calories.” Its metabolic effects also depend on the amount, form, and context in which it is consumed.

The body's satiety signals don't occur instantaneously. Eating rapidly can make it easier to consume substantial energy before fullness becomes apparent.
Ultra-processed foods also tend to have high energy density.
A relatively small amount can contain many calories.
This combination—high palatability, rapid consumption, and high energy density—can encourage overeating.
One particularly interesting study was conducted by researchers at the National Institutes of Health.
Participants lived in a research facility and were offered either an ultra-processed diet or an unprocessed diet.
The diets were designed to be similar in several nutritional characteristics, and participants were allowed to eat as much as they wanted.
On the ultra-processed diet, participants consumed roughly 500 more calories per day and gained weight. On the unprocessed diet, they ate less and lost weight.
This doesn't prove that every ultra-processed food is harmful.
But it provides unusually strong experimental evidence that the degree of food processing can influence how much people spontaneously eat.
Lustig also emphasizes the relationship between diet and the gut microbiome.
The trillions of microorganisms living in the digestive tract interact with the food we eat.
Fiber-rich whole foods provide substrates for beneficial fermentation.
A diet dominated by highly refined foods may provide fewer of those substrates.
Researchers are also studying whether certain emulsifiers, sweeteners, and other food additives affect the microbiome or intestinal barrier.
This is an active area of research, and many questions remain unresolved.
It is reasonable to be interested without assuming that every food additive has been proven harmful.
This is an important qualification.
The term ultra-processed covers a very broad range of foods.
A packaged whole-grain bread, protein product, flavored yogurt, and candy bar may all meet certain definitions of ultra-processing while having very different nutritional characteristics.
Food classification systems aren't perfect.
Protein, fiber, added sugar, calorie density, and overall dietary pattern still matter.
So the objective shouldn't necessarily be to become frightened of anything that comes in a package.
The more practical goal is to prevent highly processed products from becoming the foundation of the diet.
For decades, consumers were encouraged to choose foods labeled low-fat.
Removing fat can make foods less appealing, so manufacturers sometimes compensated with sugar, refined starch, or other ingredients.
The resulting product could be marketed as healthy because it contained less fat while still being highly processed and easy to overeat.
Lustig sees this as an example of the danger of focusing on a single nutrient instead of the entire food.
The same warning applies today to labels such as:
“Natural”
“Gluten-free”
“Plant-based”
“Keto”
“High protein”
None automatically tells us whether a food is healthy.
Lustig's answer is straightforward:
Eat real food.
That means making minimally processed foods the default:
Vegetables
Whole fruits
Fish, meat, eggs, and other protein sources
Nuts and seeds
Healthy fats
Fiber-rich foods
Minimally processed dairy, if desired
The exact diet can vary considerably.
Someone can eat a Mediterranean diet, a lower-carbohydrate diet, or another whole-food dietary pattern while following this principle.
One of the simplest ways to reduce ultra-processed food is to prepare more meals at home.
Cooking doesn't have to be elaborate.
Eggs, vegetables, fish, chicken, salads, soups, nuts, cheese, yogurt, and other basic foods can make relatively simple meals.
When you prepare the food yourself, you know what's in it.
For many people, the obstacle isn't nutritional knowledge but time and convenience.
That is why Lustig also views the problem as societal rather than purely individual.
Ultra-processed foods are inexpensive, convenient, heavily advertised, and available almost everywhere.
Whole foods often require preparation and can spoil.
For people with limited time, money, transportation, or cooking facilities, simply saying “eat better” isn't particularly helpful.
Lustig argues that public policy and the food industry therefore have roles to play.
He supports measures such as clearer labeling, reducing added sugar, improving access to real food, and limiting the marketing of unhealthy foods to children.
I don't think we need to decide that every processed food is dangerous.
Nor do we need to eat a perfect diet.
The more useful lesson is about proportion.
If most of what we eat consists of recognizable, minimally processed foods, an occasional highly processed food probably isn't the central issue.
The problem arises when ultra-processed food becomes breakfast, lunch, dinner, snacks, and beverages.

The answer won't tell us everything about nutrition.
But it can tell us quite a lot.
For me, Lustig's most useful advice can be reduced to three words:
Eat real food.
