Every brain cell requires energy to function. Much of that energy is produced by mitochondria—the structures within cells responsible for converting nutrients into usable energy.
Dr. Palmer proposes that impaired mitochondrial and metabolic function can disrupt brain networks and contribute to psychiatric symptoms. His theory attempts to connect many factors already associated with mental illness—including genetics, stress, inflammation, hormones, sleep, medications, diet, and insulin resistance—through their effects on metabolism and mitochondria.
The theory does not mean that all mental illnesses have a single cause. Rather, Palmer proposes metabolism as a framework for understanding how many different biological, psychological, and environmental influences may interact.
Dr. Palmer became interested in ketogenic diets partly because they have been used for more than a century to treat epilepsy. Some medications used to control seizures are also used to treat bipolar disorder, suggesting possible overlap between neurological and psychiatric mechanisms.
A ketogenic diet dramatically reduces carbohydrate intake and increases production of ketones. This changes the brain’s energy metabolism and may affect mitochondrial function, neurotransmitters, inflammation, and other cellular processes.
Palmer has reported striking improvements in some individual patients with serious mental illness treated with ketogenic metabolic therapy. Individual cases, however, cannot establish whether a treatment is broadly effective.
Clinical research into ketogenic therapy for psychiatric illness has expanded rapidly.
A 2024 Stanford pilot study followed 23 people with schizophrenia or bipolar disorder who also had metabolic abnormalities. After four months on a ketogenic diet, participants showed substantial improvements in metabolic health as well as improvements in several psychiatric measures.
More recently, a randomized controlled trial compared a ketogenic diet with participants’ usual diets in people with schizophrenia-spectrum or bipolar-I disorders. The ketogenic group showed improvements in several measures of metabolic health. Participants who continued the diet for four months also showed improvements in psychiatric symptoms and cognitive performance.
These findings are encouraging, but the research remains relatively small and short-term. Larger studies are needed to determine how effective ketogenic metabolic therapy is, which patients are most likely to benefit, and whether benefits persist over time.
Dr. Palmer emphasizes that metabolic health involves considerably more than food. Exercise, sleep, substance use, stress, medications, hormones, social relationships, and other factors can all affect metabolism and mental health.
His broader argument is that improving the health and energy regulation of the entire body may sometimes improve the functioning of the brain.
